NOTE: THE OPERATION HAS A HACCP PLAN ON FILE. THE OPERATOR SHALL SUBMIT HACCP AND VARIANCE REQUEST FORM TO THE OHIO DEPARTMENT OF HEALTH FOR APPROVAL.
