NEXT, SUBMIT THE FOLLOWING DOCUMENTS TO ODH FOR REVIEW AND POSSIBLE APPROVAL TO RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION: THE RECIPROCITY APPLICATION (PROVIDED DURING INSPECTION), A COPY OF THE COURSE CERTIFICATE, AND A COPY OF THE COURSE CURRICULUM (NOT A GENERIC COURSE AGENDA).
