Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. REPAIR OR REPLACE THE REACH IN COOLER DOORS TO PREVENT A POSSIBLE ACCIDENT FROM FALLING GLASS.

OBSERVED THAT THE MOP SINK IS NOT WORKING PROPERLY--THE WATER IS BEING TURNED ON FROM THE VALVE BEHIND THE UNIT.
MAKE NECESSARY REPAIRS/ADJUSTMENTS IN ORDER TO TURN THE WATER ON AND OFF FROM THE MOP SINK'S FAUCETS.

Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED UNEVEN AND BROKEN FLOORING IN THE FACILITY.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PROVIDE REPAIR TO THE FLOORING THROUGHOUT THE FACILITY IN ORDER TO MAKE THE FLOORS SMOOTH AND EASILY CLEANABLE.

The RFE license was not posted in the facility. OBSERVED THAT THE CURRENT RETAIL FOOD ESTABLISHMENT LICENSE IS NOT POSTED AT THE FACILITY.
A license holder shall display the license for that retail food establishment at all times at the licensed location. OBTAIN AND POST THE CURRENT RETAIL FOOD ESTABLISHMENT LICENSE IN A CONSPICUOUS LOCATION AT THE FACILITY.
