Critical Written procedures and plans are not maintained and implemented. THE OPERATION DID NOT HAVE A HACCP PLAN ON FILE FOR USING A REDUCED OXYGEN PACKAGING METHOD FOR TCS FOODS. THE OPERATION SHALL NOT USE THE CRYOVAC MACHINE UNTIL A HACCP PLAN IS ON FILE AND REVIEWED BY THE HEALTH DEPARTMENT.

Critical Wild mushrooms from an unapproved source. OBSERVED WILD MUSHROOMS AT THE FOODS SERVICE OPERATION AND DOCUMENTATION OF APPROVED SOURCES WERE NOT PROVIDED AT TIME OF INSPECTION. FOOD SHALL BE OBTAINED FROM SOURCES THAT COMPLY WITH THE LAW. THE WILD MUSHROOMS WERE DISCARDED AT TIME OF INSPECTION.

SPOKE TO THE OPERATOR REGARDING A FORAGED MUSHROOM COMPLAINT. THE OPERATOR STATED THAT WILD FORAGED MUSHROOMS ARE NOT SERVED/SOLD AT THE FOOD ESTABLISHMENT. FOOD SHALL BE OBTAINED FROM SOURCES THAT COMPLY WITH THE FOOD LAW. THE OPERATOR STATED THAT THE CHICKEN OF THE WOODS MUSHROOMS (LAETIPORUS) ARE SERVED AT THE FOOD SERVICE ESTABLISHED. THE OPERATOR DID NOT PROVIDE DOCUMENTATION OF APPROVED SOURCES AT TIME OF INSPECTION. THE CHICKEN OF THE WOODS MUSHROOMS ARE USED IN THE MUSHROOM-BEAN PATTIES. THE MUSHROOM-BEAN PATTIES AND MUSHROOMS WERE DISCARDED BY THE OPERATOR.THE OPERATOR HAS A WILD MUSHROOM IDENTIFICATION/FORAGING LICENSE FROM MICHIGAN. THE OPERATOR ALSO CONDUCTS A WILD MUSHROOM IDENTIFICATION BUSINESS AT THE FOOD SERVICE OPERATION. OBSERVED WILD MUSHROOMS STORED ON A METAL TRAY IN THE KITCHEN. THE OPERATOR INDICATED THAT THE MUSHROOMS WERE NOT FOR SERVICE/SALE, AND WERE BEING EXAMINED TO IDENTIFY THE SPECIES FOR A CUSTOMER. THE OPERATOR WAS ADVISED NOT TO CONDUCT WILD MUSHROOM IDENTIFICATION SERVICES AT THE FOOD SERVICE ESTABLISHMENT.

THE OPERATION PRODUCES MISCELLANEOUS JARRED/CANNED/FERMENTED FOOD ITEMS (TOMATO BASED SAUCES, FERMENTED VEGETABLES, VARIOUS SAUCES). THE MISCELLANEOUS FOOD ITEMS ARE STORED ON FOOD STORAGE SHELVES ON THE SALES FLOOR. THE OPERATOR STATED THAT THE FOOD ITEMS ARE FOR DISPLAY ONLY AND NOT FOR SALE. THE OPERATOR AFFIRMED THAT FOODS SOLD TO CONSUMERS ARE NOT CURED. THE OPERATION SHALL NOT SELL CURED/JARRED/CANNED/FERMENTED FOOD ITEMS WITHOUT AN APPROVED VARIANCE FROM OHIO DEPARTMENT OF HEALTH.

OBSERVED A DRY AGER COOLER THAT HELD CURED/FERMENTED RAW ANIMAL PRODUCTS. THE OPERATOR STATED THAT THE ITEMS IN THE COOLER ARE NOT FOR SALE AND PART OF AN EXPERIMENT FOR A RETAIL FOOD ESTABLISHMENT (R AND D'S).
