Critical Repeat The person in charge was unable to demonstrate proper knowledge of food safety and prevention. OBSERVED CRITICAL VIOLATIONS AT THE FACILITY WHICH DEMONSTRATE A LACK OF FOOD SAFETY KNOWLEDGE.
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection. 3717-1-03.4(D) / Cooling - temperature and time control.
Critical Corrected During Inspection TCS foods were not cooled using the proper time and temperature parameters. OBSERVED SCRAMBLED EGGS THAT WERE STILL 79 DEGREES F AFTER BEING PUT IN THE COOLER AT 9 AM AND IT WAS AFTER 1PM WHEN THEY WERE TEMPED.
To prevent the growth of pathogens, cooked TCS food shall be rapidly cooled: within two hours from 135°F to 70°F, and within a total of six hours from 135°F to 41°F or less; (2) TCS food shall be cooled within 4 hours to 41°F or less if prepared from ingredients at ambient temperature, such as reconstituted foods and canned tuna; (3) TCS food received in compliance with laws allowing a temperature above 41°F during shipment from the supplier, shall be cooled within 4 hours to 41°F or less; (4) Raw eggs shall be received at 45°F or less and immediately placed in refrigerated equipment that maintains an ambient air temperature of 45°F or less. 3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. OBSERVED NO AIR GAP FOR THE STEAMER, ICE MACHINE, WAREWASHER, OR CONDENSATION LINES FOR THE COOLER.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED NO EMPLOYEE WITH A LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. 3717-1-03.1(A)(3) / Sources - packaged food labeling
Repeat Observed food packages that are not properly labeled. OBSERVED NICHOLS BREAD WITH NO LABELS.
To properly inform consumers, packaged food shall be labeled as specified in law.
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Fixed equipment did not appear to be properly sealed or spaced for cleaning. OBSERVED 3 COMPARTMENT SINK AND COUNTERTOP BY WAREWASHER NOT SEALED TO THE WALL.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. OBSERVED OVENS WITH BENT FLOORS, A LEAKING STEAMER, AND A DRIPPING COFFEE MACHINE.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-05.4(H) / Toilet room receptacle - covered.
Repeat Observed no covered receptacle in women's restroom. OBSERVED NO COVERED RECEPTACLE IN THE WOMEN'S BATHROOM.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Observed an improperly cleaned storage area for refuse, recyclables, or returnables. OBSERVED FOOD AND GARBAGE ON THE GROUND IN FRONT OF THE DUMPSTER.
A storage area and enclosure for refuse, recyclables, or returnables shall be maintained clean and free of unnecessary items.
3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed light bulbs or heat lamp that were not properly shielded or coated where required. OBSERVED LIGHT SHIELD SLEEVES BUT NO ENDCAPS ON THE LIGHTS.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. OBSERVED CRACKED OR MISSING WALL TILES.
The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris. OBSERVED THE HOOD FILTERS HAD AN ACCUMULATION OF GREASE.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
