Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. OBSERVED A BUILD UP OF DEBRIS ON THE INSIDE OF THE BAR GUN DISPENSER. TO LIMIT THE GROWTH OF PATHOGEN THAT MAY CAUSE A FOODBORNE ILLNESS, FOOD CONTACT SURFACES SHALL BE CLEANED TO SIGHT AND TOUCH.

Critical Repeat Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. OBSERVED THE QUATERNARY SANITIZING SOLUTION SHALL MEET THE MINIMUM REQUIREMENT FOR CONCENTRATION AT THE THREE COMPARTMENT SINK (BAR). TO LIMIT THE GROWTH OF PATHOGENS THAT MAY CAUSE A FOODBORNE ILLNESS, SANITIZING SOLUTION SHALL MEET THE MINIMUM REQUIREMENTS FOR EFFECTIVE SANITIZING.

Repeat Facility does not have an employee with manager certification in food protection. OBSERVED NO MANAGER/SUPERVISOR WITH AN OHIO FOOD MANAGERS CERTIFICATION.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. OBSERVED THE HANDLE OF THE ICE SCOOPERS IN DIRECT CONTACT WITH THE ICE IN THE ICE WELLS AT THE BAR.

Provided PIC (Person in Charge) with various food safety handouts. The operation is a risk level III and practicing risk level IV activities (reheating in bulk). The operation shall upgrade their food license to a risk level IV.
