[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. THE MANAGER LEVEL FOOD SAFETY TRAINING COURSE CERTIFICATE WAS OBTAINED BY THE PERSON IN CHARGE. THE RECIPROCITY APPLICATION TO SEEK THE LEVEL TWO CERTIFICATION WAS PROVIDED DURING THE TIME OF INSPECTION. SUBMIT ALL REQUIRED DOCUMENTS (AS STATED ON THE APPLICATION) TO THE OHIO DEPARTMENT OF HEALTH FOR REVIEW. UPON APPROVAL, THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION WOULD BE SENT TO THE PERSON IN CHARGE.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED THAT THE RIGHT SIDE REACH IN COOLER IN THE STORAGE ROOM NEXT TO THE MAIN KITCHEN HAS AN AMBIENT TEMPERATURE OF 49 DEGREES FAHRENHEIT.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. IMMEDIATELY DISCONTINUE USING THIS UNIT UNTIL IT HAS BEEN REPAIRED/SERVICED AND CAN MAINTAIN AN AMBIENT TEMPERTURE NO HIGHER THAN 41 DEGREES FAHRENHEIT. ONCE SERVICED, CONTACT THE CLEVELAND HEALTH DEPARTMENT TO HAVE THE UNIT VERIFIED FOR APPROVAL TO RE-USE.
