FOODS ARE RECEIVED FROM APPROVED SOURCES: VISTAR, J AND J SNACKS. PROBE THERMOMETER-YES. SANITIZER- STERAMINE. TEST KIT-.PEST CONTROL-SPEEDY. PROVIDED THE PERSON IN CHARGE WITH AN EMPLOYEE HEALTH POLICY, A VOMIT/FECAL CLEAN UP, AND VARIOUS FOOD SAFETY HANDOUTS. THE OPERATOR SHALL OBTAIN A LEVEL II CERTIFICATION WITHIN 30 DAYS. OBSERVED A STICKY SUBSTANCE ON THE FLOOR UNDER AND AROUND THE BEVERAGE DISPENSER EQUIPMENT. OBSERVED A STICKY SUBSTANCE ON THE BASEMENT FLOOR (UNDER THE KITCHEN). THE OPERATOR SHALL NOT USE THE BEVERAGE DISPENSER EQUIPMENT UNTIL IT IS IN GOOD WORKING ORDER. THE OPERATOR SHALL HAVE THE PLUMBING REPAIRED WITH IN 24 HOURS. THE OPERATOR SHALL NOT USE THE ICE MACHINE UNTIL IT IS PROPERLY CLEANED AND SANITIZED.
