Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION WAS NOT OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN APPROVED FIFTEEN HOUR FOOD SAFETY TRAINING COURSE, OBTAIN THE CERTIFICATE UPON COURSE COMPLETION, THEN CONTACT THE OHIO DEPARTMENT OF HEALTH FOR ADDITIONAL INSTRUCTIONS TO OBTAIN THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.

Observed no covered receptacle in women's restroom. OBSERVED THAT THE LADIES' RESTROOMS DO NOT HAVE COVERED RECEPTACLES INSIDE.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. PROVIDE THE LADIES' RESTROOM WITH COVERED RECEPTACLES FOR SANITARY NAPKIN DISPOSAL (THE MANAGER STATES THAT THE COVERED RECEPTACLE HAVE BEEN ORDERED AND SHOULD ARRIVE TO THE FACILITY SOON).

Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED BROKEN FLOOR TILE ON THE BAKERY DEPARTMENT FLOOR NEAR THE WALK IN COOLER AND WALK IN FREEZER.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE ALL BROKEN FLOOR TILE ON THE BAKERY DEPARTMENT FLOOR AND THROUGHOUT THE FACILITY AS NEEDED IN ORDER FOR IT TO BE SMOOTH AND EASILY CLEANABLE.

Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPAIR THE MISSING AND CRACKED WALL TILE IN THE SEAFOOD SECTION IN ORDER FOR IT TO BE SMOOTH AND EASILY CLEANABLE.
