3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED ICE BUILD-UP IN ONE OF THE WALK IN COOLER CONDENSERS.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. MAKE NECESSARY REPAIRS/ADJUSTMENTS TO THE WALK IN COOLER CONDENSER IN ORDER FOR IT TO OPERATE PROPERLY.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. OBSERVED THREE (3) MISSING FAN GUARDS INSIDE OF THE WALK IN COOLER.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. REPLACE ALL THREE (3) MISSING FAN GUARDS TO PREVENT A POSSIBLE HAZARD.

Non-food contact surfaces of equipment are dirty. OBSERVED DIRT BUILD-UP ON TWO (2) FAN GUARDS INSIDE OF THE WALK IN COOLER.
Nonfood-contact surfaces of equipment shall be kept clean. THOROUGHLY CLEAN THE TWO (2) FAN GUARDS INSIDE OF THE WALK IN COOLER.

The plumbing system was not properly maintained. OBSERVED THAT THE COLD WATER FAUCET IS NOT WORKING AT THE MOP SINK.
A plumbing system shall be properly maintained. MAKE NECESSARY REPAIRS/ADJUSTMENTS TO MAKE THE COLD WATER OPERABLE FROM THE MOP SINK FAUCET.

Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED THAT THERE ARE MISSING CEILING TILES IN THE FACILITY.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPLACE ALL MISSING CEILING TILES THROUGHOUT THE FACILITY.

Repeat Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT, LOOSE CARDBOARD AND BOTTLE CAPS ON THE FLOOR BELOW THE WALK IN COOLER RACKS.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. REMOVE ALL DEBRIS AND THOROUGHLY CLEAN THE ENTIRE FLOORING (INCLUDING THE FLOORING BELOW ALL WALK IN COOLER RACKS) INSIDE OF THE WALK IN COOLER TO PREVENT POSSIBLE INFESTATION AND HARBORAGE OF PESTS/RODENTS.
3717-1-06.4(B) / Cleaning - frequency and restrictions.

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
The ventilation system was not being properly cleaned or was creating a public health hazard. OBSERVED A BUILD-UP OF DIRT ON THE CEILING VENTS.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. THOROUGHLY CLEAN ALL CEILING VENTS THROUGHOUT THE FACILITY.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED NON-FUNCTIONAL AND/OR UNNECESSARY EQUIPMENT: AN ICE MACHINE, DEEP FREEZER, MICROWAVE, THREE DOOR REACH IN COOLER, GASOLINE CANS AND EXTENSION CORDS STORED IN THE REAR OF THE FACILITY.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE ALL NON-FUNCTIONAL EQUIPMENT FROM THE FACILITY TO PREVENT POSSIBLE INFESTATION AND POSSIBLE HARBORAGE FROM PESTS/RODENTS.
901:3-4-02(G) / RFE must display license
The RFE license was not posted in the facility. OBSERVED THAT THE CURRENT RETAIL FOOD ESTABLISHMENT LICENSE HAS NOT BEEN OBTAINED.
A license holder shall display the license for that retail food establishment at all times at the licensed location. OBTAIN AND POST THE CURRENT RETAIL FOOD ESTABLISHMENT LICENSE.
