III - Preventing Contamination by Hands: The hand washing sink(s) were not accessible for convenient use by employees. OBSERVED THERE IS NO HANDWASHING SINK IN THE KITCHEN. THE ONLY HANDWASHING SINKS AVAILABLE ARE IN THE BATHROOMS IN THE HALL. IN ORDER TO PREVENT CONTAMINATION AND PROMOTE GOOD HANDWASHING, A HANDWASHING STATION SHOULD BE AVAILABLE IN THE KITCHEN FOR CONVENIENT USE.

VII - Protection from Contamination: Equipment food-contact surfaces or utensils are dirty. OBSERVED THE CAN OPENER BLADE WAS SOILED. IN ORDER TO PREVENT CONTAMINATION, THE CAN OPENER BLADE SHOULD BE CLEANED EVERY FOUR HOURS WHILE IN USE OR AFTER ITS FINAL USE OF THE DAY IN ORDER TO MAINTAIN IT CLEAN TO SIGHT AND TOUCH. PIC INSTRUCTED TO CLEAN CAN OPENER BLADE.

VI - P - TCS Food: Observed hot foods being held at 135°F or above; cold foods being held at 41°F or below. OBSERVED MEATBALLS AND MARINARA SAUCE HOT HOLDING AT 135 DEGREES F OR ABOVE AND TUNA SALAD, SAUSAGE, AND TOMATOES HOLDING AT 41 DEGREES F OR BELOW.

X - P - Chemical: Toxic materials are properly identified and stored. OBSERVED ALL TOXIC CHEMICALS STORED AWAY FROM DISHWARE, UTENSILS, AND FOOD.
