3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. OBSERVED THAT THE COUNTERTOP WHERE THE MICROWAVE AND PIZZA OVEN ARE LOCATED ON HAS A BUILD-UP OF DIRT.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 05-Aug-2015
3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Food equipment surfaces are not cleaned at the required frequency.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule. THE THREE COMPARTMENT SINK IS CURRENTLY DOWN DUE TO THE FAUCET BREAK. DUE TO THIS ISSUE, IT DOES NOT ALLOW THE CAPPUCCINO MACHINE TO BE CLEANED AT THE REQUIRED FREQUENCY BEING THAT THE MACHINE IS REQUIRED TO BE CLEANED USING THE THREE COMPARTMENT SINK. 3717-1-07.2 / Poisonous or toxic materials: storage and display - separation
Critical Observed improper display or storage of poisonous or toxic materials offered for retail sale. OBSERVED ALUMINUM FOOD STORAGE PANS PLACED BELOW BLEACH AND PINE-SOL CLEANER.
To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, single-service articles, or single use articles by separating the poisonous or toxic materials, and locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles, or single-use articles. THE ITEMS WERE PLACED IN A DIFFERENT LOCATION AWAY FROM POISONOUS OR TOXIC MATERIALS AT THE TIME OF INSPECTION. Correct By: 30-Jul-2015

3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
The plumbing system was not properly repaired. OBSERVED THAT THE THREE COMPARTMENT SINK DOES NOT HAVE A FAUCET. THE MANAGER SHOWED THE UNIT TO ME AND EXPLAINED THAT IT FELL OFF FROM THE SINK.
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom. OBSERVED THAT THERE WAS NO RECEPTACLE LID IN THE UNISEX RESTROOM.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. OBSERVED A BROKEN FLOOR COVER IN THE WALK IN COOLER.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.1(K) / Insect control devices - design and installation.
Observed improper use or placement of an insect control device. OBSERVED "STICKY TAPE" PLACED BETWEEN THE COOKING EQUIPMENT AND THE THREE COMPARTMENT SINK.
Insect control devices that stun insects shall be designed to retain the insect within the device. Insect control devices shall be installed so that: they are not located over a food preparation area; and dead insects and insect fragments are prevented from being impelled onto or falling on exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT ON THE RESTROOM FLOOR.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED FOOD PARTICLES (PEANUTS) ON THE FLOOR INSIDE OF THE WALK IN COOLER.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Observed the presence of dead pests or insects. OBSERVED DEAD INSECTS ON FOUR ALUMINUM PANS.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. THE ALUMINUM PANS WERE REMOVED AND THROWN AWAY AT THE TIME OF INSPECTION
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED LITTER AND DEBRIS (INCLUDING A LARGE PIECE OF WOOD) AROUND THE DUMPSTER.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
