Critical Repeat Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Observed no employee health policy on file for food employees. The operator shall require food employees to report information about their health as it relates to illness that can be transmitted through food.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. Observed a build up of debris on the following food contact surfaces: - Meat Slicer - Potato Slicer - Can opener - Utensils stored on the utensil rack - Cutting board on the prep coolers - Coffee pots To limit the growth of pathogens that may cause a foodborne illness, food contact surfaces shall be clean to sight and touch.

To limit the growth of pathogens that may cause a foodborne, TCS foods shall be datemarked if held more than 24 hours and used/discarded within 7 days of the preparation date.

Critical Corrected During Inspection Presence of live insects, rodents, and other pests. Observed live flying insects throughout the facility. The presence of pest shall be minimize to control their presence on the premises.

Repeat Facility does not have an employee with manager certification in food protection. Observed no manager/supervisor with an Ohio food managers certification.

THE OPERATION VOLUNTARILY CLOSED FOR CLEANING FOR 48 HOURS.
Critical Control Point Inspection
VI - TCS Food: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. Observed TCS foods not datemarked throughout the facility. To limit the growth of pathogens that may cause a foodborne illness, TCS foods shall be datemarked if held more than 24 hours and used within 7 days of the preparation date.
IV - Demonstration of Knowledge: The person in charge was unable to demonstrate proper knowledge of food safety and prevention. Observed critical violations at time of inspection. The operator shall maintain control and oversight of the facility to ensure that critical violations do not occur.
