Critical Corrected During Inspection Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired. OBSERVED EXPIRED SIMILAC FORMULA. 3 BOTTLES WERE DISCARDED.

Repeat Equipment components are not intact, tight or properly adjusted. OBSERVED MASTERBILT WALL COOLER DOOR #6 TO BE TAPED OVER GASKET, PROHIBITING A FULL SEAL.

Non-food contact surfaces of equipment are unclean. OBSERVED CONDENSER FAN GUARDS AND CEILING OF WALK-IN COOLER TO HAVE ACCUMULATION OF DUST.

Repeat Floors, walls, and/or ceilings not smooth and easily cleanable. OBSERVED FLOOR TILES IN WALK-IN COOLER TO BE MISSING, CREATING A SURFACE THAT IS NO LONGER SMOOTH AND EASILY CLEANABLE.
