To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection. Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. OBSERVED RAW MEAT STORED OVER READY TO EAT FOOD IN THE WALK IN COOLER.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. IN ORDER TO PREVENT CONTAMINATION, STORE RAW MEATS UNDER READY TO EAT FOODS. PIC WAS GIVEN A HANDOUT ON THE PROPER STORAGE HIERACHY FOR FOODS.
3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, using chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection The concentration and/or temperature of a chlorine sanitizing solution did not meet the minimum requirements specified in the code. OBSERVED THE LOW TEMP WAREWASHING MACHINE WITH NO CHLORINE SANITIZER. THE LINES IN THE MACHINE WERE ALSO EMPTY AND THE MACHINE HAD TO BE RUN SEVERAL TIMES BEFORE SANITIZER COULD BE DETECTED.
A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution as specified in the code. IN ORDER TO ENSURE THE WAREWASHER IS FUCTIONING PROPERLY AND SANITIZING ALL DISHWARE, CHECK THE SANITIZER FREQUENTLY.
3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection A quaternary ammonium sanitizing solution did not meet the minimum requirements for temperature, concentration, and/or water hardness. OBSERVED THE THREE COMPARTMENT SINK WITH NO SANITIZER.
A quaternary ammonium sanitizing solution shall: (a) Have a minimum temperature of 75F,(b) Have a concentration as specified and as indicated by the manufacturer's use directions included in the labeling, (c) Be used only in water with 500 ppm (mg/l) hardness or less, or used in water having a hardness no greater than specified by the manufacturer's label. IN ORDER TO PREVENT CONTAMINATION, FREQUENTLY CHECK SANITIZER AT THREE COMPARTMENT SINK.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are dirty. OBSERVED MEAT SLICER WITH FOOD DEBRIS, MIXER WITH FOOD DEBRIS, A SOILED CAN OPENER, AND A DIRTY STRAINER HUNG ON A UTENSIL RACK.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. OBSERVED NO AIR GAP ON STEAMER, THE ICE MACHINE, THE WAREWASHER, AND THE CONDENSATION LINES FOR THE WALK-IN COOLER.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. IN ORDER TO PREVENT CONTAMINATION, PROVIDE AIR GAPS FOR PLUMBING FOR THE ICE MACHINE, STEAMER, WAREWASHER, AND CONDENSATION LINES FOR THE WALK-IN COOLER. Correct By: 19-Jan-2018

3717-1-02.2(G) / Jewelry - prohibition.
Observed food employees wearing jewelry on arms or hands during food preparation. OBSERVED AN EMPLOYEE WEARING A WEDDING RING WITH A DIAMOND AND AN EMPLOYEE WEARING A BRACELET.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands. IN ORDER TO PREVENT FOOD CONTAMINATON, EMPLOYEES ARE ONLY PERMITTED TO WEAR A PLAIN WEDDING BAND OR MEDICAL ALERT BRACELET PLACED HIGH UP ON THE ARM.
3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED NO EMPLOYEE WITH A LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. OBSERVED NO VOMITING AND DIARRHEAL CLEAN UP POLICY.
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-03.1(A)(3) / Sources - packaged food labeling
Observed food packages that are not properly labeled. OBSERVED NICHOLS BREAD BOTH SINGLE SLICES AND LOAVES WITH NO LABELS AT ALL.
To properly inform consumers, packaged food shall be labeled as specified in law.
3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection In-use utensils are improperly stored. OBSERVED THE BULK SUGAR SCOOPER WITH THE HANDLE LAYING IN THE SUGAR IN THE DRY STORAGE AREA.
During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored as described in this rule.
3717-1-04.1(K) / Food temperature measuring devices - accuracy.
Food thermometers are not accurate to plus or minus one degree C, or plus or minus two degrees F in the intended range of use. OBSERVED THE FOOD THERMOMETER WAS NOT CALIBRATED AND ABLE TO TAKE ACCURATE TEMPERATURES.
To prevent foodborne illness, food temperature measuring devices that are scaled only in Celsius or dually scaled in Fahrenheit and Celsius shall be accurate to plus or minus one degree Celsius in the intended range of use; Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to plus or minus two degrees Faranheit in the intended range of use.
3717-1-04.2(G)(2) / Food temperature measuring devices - measuring the temperature of thin foods.
A thermometer capable of accurately measuring the temperature of thin foods was not available. OBSERVED NO SMALL DIAMETER PROBE IN THE FACILITY.
To prevent foodborne illness, a food temperature measuring device with a suitable small-diameter probe shall be provided and readily accessible to measure the temperature of thin foods, such as meat patties or fish fillets.
3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment did not appear to be properly sealed or spaced for cleaning. OBSERVED 3 COMPARTMENT SINK AND COUNTERTOP BY WAREWASHER NOT SEALED TO THE WALL.
Equipment that is fixed shall be installed so that it is spaced to allow access for cleaning, from adjoining equipment, walls, and ceilings a distance of not more than one thirty-second inch or one millimeter; or sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage; Counter-mounted equipment shall be installed to allow cleaning of the equipment and areas underneath and around the equipment as specified in this rule.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED A CHIPPED BLADE ON THE FOOD PROCESSOR MACHINE.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. PIC WAS INSTRUCTED TO TAKE THE FOOD PROCESSOR OUT OF SERVICE UNTIL A NEW BLADE COULD BE PURCHASED.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED OVENS WITH BENT FLOORS, THE STEAMER WAS LEAKING AND HOLDING WATER, THE COFFEE MACHINE WAS DRIPPING AND NEEDED A CATCH BASIN TO CONTROL THE LEAKING, AND ONE OVEN/GRILL TOP OUT OF SERVICE.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. OBSERVED BROKEN GASKET ON THE REACH IN COOLER IN THE KITCHEN AREA.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. OBSERVED ALL NON CONTACT FOOD SURFACES ON EQUIPMENT WAS DIRTY INCLUDING THE OVENS, WAREWASHER, AND STEAMER. IN ADDITION, OBSERVED DUST ACCUMULATION IN THE WALK IN COOLER ON THE CEILING AND CONDENSATION LINES.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. OBSERVED FOOD DEBRIS IN AREAS WHERE CUTTING BOARDS AND FOOD CONTAINERS WERE STORED.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-04.8(A) / Equipment and utensils - air-drying required.
Equipment and utensils are not being air dried. OBSERVED STORED FOOD CONTAINERS AND PANS THAT WERE STILL WET.
After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food and utensils, and may not be cloth dried.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Single-service or single-use articles, kitchenware, or tableware were not handled, displayed, or dispensed properly. OBSERVED SCOOPERS STORED IN A RACK ON THE WALL THAT DID NOT PROHIBIT CONTAMINATION OF FOOD CONTACT SURFACES BY HANDS WHEN THEY WERE TAKEN OUT TO BE USED.
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no covered receptacle in women's restroom. OBSERVED NO COVERED RECEPTACLE IN THE EMPLOYEE BATHROOM FOR SANITARY NAPKINS.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.1(A) / Floors, walls, and ceilings.
Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED SEVERAL WALLS WITH HOLES.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. OBSERVED ALL LIGHTS IN THE FACILITY HAD LIGHT SHIELD SLEEVES ON THEM BUT NO ENDCAPS.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. OBSERVED AN OPEN WINDOW IN THE KITCHEN WITH A SCREEN ONLY ON ONE SIDE. THE WINDOW WAS OPEN ON THE SIDE WITH NO SCREEN.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. OBTAIN A SCREEN SO THAT BOTH SIDES OF THE WINDOW ARE PROTECTED FROM INSECT ENTRY.
3717-1-06.2(B) / Handwashing cleanser - availability.
Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s). OBSERVED NO SOAP IN THE EMPLOYEE BATHROOM.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Corrected During Inspection Observed no towels or hand drying device at the handwashing sink(s). OBSERVED NO PAPER TOWELS AT THE HANDWASHING SINK.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWASHING SIGN AT THE HANDWASHING SINK IN THE KITCHEN.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.2(I)(1) / Lighting - intensity (10)
The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. OBSERVED LIGHT INTENSITY OF 4 FOOT CANDLES IN THE WALK IN COOLER.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
3717-1-06.2(I)(3) / Lighting - intensity (50)
The light intensity in an area where an employee was working with food, utensils or equipment was less than fifty foot candles. OBSERVED LIGHT INTENSITY ABOVE THE GRILL AREA AT 39 FOOT CANDLES.
The light intensity shall be at least fifty foot candles (five hundred forty lux) at a surface where a food employee is working with food, utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. OBSERVED WALL TILES THAT ARE CRACKED OR MISSING, PAINT PEELING, AND FLOOR TILES MISSING.
