Critical Date the last shellstock was sold or served was not recorded on the label. Observed in the seafood department the date of the last sell of shellstock is not being recorded on the shellstock tag.

Critical TCS foods not being hot held at the proper temperature. Observed chicken wing portions being held in the hot display case and they were not being held at 135F or above. All hot foods must be kept at 135F or above.

Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed bbq ribs and chicken being held on a display island and were temping at 60F.

Critical Corrected During Inspection Equipment and/or utensils improper construction. Observed raw fish being stored in the cooler inside of a box (with no plastic covering on the box top) for a customer order. Food contact surfaces must be smooth and easily cleanable.

Improper storage of food items. Observed ready to eat food items in the meat department and in the bakery that were stored on speed racks but did not have protective coverings for the top tray of items. Please ensure that ready to eat foods are protected from possible contamination.

3717-1-02.3(C) / Hair restraints - effectiveness.
Food employee(s) hair not effectively restrained. Observed employees in different departments not wearing hair restraints and some once they applied hair nets were not properly being worn. Please ensure that any employees working with food are properly wearing hairnets or some type of hair restraint. Effective March 2019 Ohio Uniform Food Safety Code was updated and it states "Food employees shall effectively restrain hair by wearing hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and work to effectively prevent their hair from contacting exposed food"

Non-food contact surfaces of equipment are unclean. Observed in the bakery that the mixing machine was heavily soiled. Please have the mixer cleaned.

Repeat Facility not maintained clean. Observed that the bakery needed to be cleaned especially near the handsink and large mixing area. Also ceiling tiles in the bakery are soiled and need to be replaced.

No protective shielding on lights. Observed in the bakery a missing light shield and the bulbs were not shatterproof.
