Critical Repeat Corrected During Inspection Handwashing sink is being used for purposes other than handwashing. OBSERVED AN EXTENSION CORD AND OTHER ITEMS INSIDE OF THE KITCHEN HANDSINK.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN APPROVED FIFTEEN HOUR FOOD SAFETY COURSE APPROVED BY THE OHIO DEPARTMENT OF HEALTH (ODH). OBTAIN THE CERTIFICATE UPON COURSE COMPLETION, THEN COMPLETE THE RECIPROCITY APPLICATION (PROVIDED DURING TIME OF INSPECTION), SUBMIT A COPY OF THE COURSE CERTIFICATE AND A COPY OF THE COURSE CURRICULUM (NOT AGENERIC AGENDA) TO SEEK APPROVAL. UPON APPROVAL, ODH WILL SEND THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed improper storage of food items. OBSERVED A BAG OF ONIONS STORED ON THE KITCHEN FLOOR.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. THE BAG OF ONIONS WAS REMOVED AND PLACED ON A CRATE DURING THE TIME OF INSPECTION. Correct By: 27-Jul-2018

Observed food stored in an unapproved location. OBSERVED BULK, PREPACKAGED POLISH SAUSAGE, CUT CHICKEN STORED INSIDE OF THE FOUR DOOR REACH IN FREEZER WHERE ITEMS ARE ACCESSIBLE TO THE PUBLIC.
To prevent contamination, food may not be stored in areas described in this rule. TO PREVENT POSSIBLE TAMPERING AND POSSIBLE CONTAMINATION, REMOVE ALL ITEMS THAT ARE USED IN FOOD PREPARATION TO THE PUBLIC AND STORE THEM IN A UNIT THAT IS PROHIBITED TO THE PUBLIC.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED DAMAGED FLOORING AND MISSING FLOOR TILES.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPAIR ALL DAMAGED FLOORING AND MISSING FLOOR TILES THROUGHOUT THE FACILITY.
3717-1-06.2(E) / Handwashing signage.
Corrected During Inspection Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWASHING SIGN AT THE KITCHEN HANDWASHING SINK.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. A HANDWASHING SIGN WAS PLACED OVER THE KITCHEN HANDWASHING SINK DURING THE TIME OF INSPECTION. Correct By: 27-Jul-2018
3717-1-06.2(B) / Handwashing cleanser - availability.
Repeat Corrected During Inspection Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s). OBSERVED NO HANDWASHING SOAP AT THE KITCHEN HANDWASHING SINK.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. HANDWASHING SOAP WAS PROVIDED AT THE KITCHEN HANDWASHING SINK DURING THE TIME OF INSPECTION. Correct By: 27-Jul-2018
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT ON THE FLOORS THROUGHOUT THE FACILITY.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. THOROUGHLY CLEAN ALL FLOORS THROUGHOUT THE FACILITY TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED A COUCH AND TIRES IN THE PARKING LOT IN BETWEEN THE GARBAGE AND GREASE DUMPSTERS.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE ANY/ALL UNNECESSARY ITEMS THROUGHOUT THE PARKING LOT TO PREVENT POSSIBLE PEST/RODENT HARBORAGE.
3717-1-07.1(P) / Personal care items - storage.
Corrected During Inspection Observed personal care items that were not stored in a designated employee storage area. OBSERVED PERSONAL TOILETRIES (BAR SOAP, DEODORANT, SHAMPOO BOTTLES) LOCATED ON THE SHELF ABOVE THE KITCHEN'S THREE COMPARTMENT SINK.
Employees shall store their personal care items in facilities as specified under (K)(2) of rule 3717-1-6.2 of this code. ALL ITEMS WERE REMOVED DURING THE INSPECTION. PLACE ALL PERSONAL CARE ITEMS IN A DESIGNATED EMPLOYEE AREA, NOT INSIDE OF THE KITCHEN OR ANY OTHER FOOD PREP AREA. MAINTAIN AT ALL TIMES. Correct By: 27-Jul-2018
