Critical Repeat Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. PIC WAS NOT AWARE OF EMPLOYEES ILLNESS REPORTING REQUIREMENTS.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food. LITERATURE WAS PROVIDED AND EXPLAINED.
3717-1-05.1(D) / Backflow prevention - air gap.
Critical Repeat Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. OBSERVED ICE MACHINE WITH NO AIR-GAP
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters). Correct By: 08-Jun-2018

Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces. LITERATURE WAS PROVIDED AND EXPLAINED.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
There is no test kit available for measuring the concentration of the sanitizer. OBSERVED: THE FACILITY HAD NO TEST KIT.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. OBSERVED BROKEN COOLER DOOR.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Equipment components are not intact, tight or properly adjusted. OBSERVED SEALS ON COOLER AND FREEZER TAPED.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. OBSERVED ITEMS STORED IN THE REFRIGERATOR WAS NOT DATE MARKED.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. OBSERVED BUILD-UP OF DIRT AND DEBRIS ON EQUIPMENT IN THE KITCHEN.
Nonfood-contact surfaces of equipment shall be kept clean. CORRECT BY: CLEAN ON A ROUTINE BASIS.
3717-1-04.6(B)/Sanitizing frequency of utensils and food-contact surfaces
Utensils and food-contact surfaces of equipment were not sanitized at the required frequency. OBSERVED PIC DID NOT HAVE THE PRODUCTS TO CONDUCT SANITIZING OPERATIONS.
To prevent pathogen growth, utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning. PIC SHALL PURCHASE CHEMICALS AND TEST STRIPS NEEDED TO SANITIZE EQUIPMENT AND UTENSILS WITHIN THE FACILITY.
