| This Retail Food Establishment is observed by CDPH to be fully in operation as a cease and desist is currently being forwarded for the failure to properly renew the Retail Food Establishment license. CDPH informs staff to Immediately discontinue all Retail Food Service operations to the Public until a current Food Service Operation license has been attained. Retail Food Establishment License renewals are available for processing at: Division of License and Assessments located at 601 Lakeside Avenue room 122, Cleveland 44114. In regards to complaint investigation CDPH informed by staff, the owner, the City of Cleveland cashed his check in early March 2020 in regards to his timely renewal license application submittal. In addition, in regards to the unsanitary conditions of the cutting board CDPH observed minor build-up which staff is communicating is continuously cleaned and sanitized within every four hours of use.
