Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. OBSERVED AN EMPLOYEE HEALTH POLICY THAT IS NOT SUFFICIENT. IN ORDER TO PREVENT THE SPREAD OF FOODBORNE ILLNESS, ALL EMPLOYEES MUST READ AND SIGN THE EMPLOYEE HEALTH POLICY WHICH CONTAINS THE COMMON FOODBORNE ILLNESS SYMPTOMS, THE 13 REPORTABLE DISEASES, AND A STATEMENT OF ACKNOWLEDGEMENT. THE FACILITY ONLY STATES 5 REPORTABLE DISEASES. THE FACILITY WILL BE PROVIDED WITH A COMPLETE EMPLOYEE HEALTH POLICY TO USE AS THEIR OWN VIA EMAIL. PLEASE HAVE ALL EMPLOYEES READ AND SIGN THE POLICY AND KEEP THEM ON FILE.

OBSERVED THE PERSON IN CHARGE DOES NOT HAVE A MANAGERS CERTIFICATION, ONLY A TACO BELL ISSUED SERVSAFE WAS PROVIDED. THE PIC WILL BE GIVEN THE APPLICATION FOR RECIPROCITY VIA EMAIL IN ORDER TO OBTAIN THE MANAGERS CERTIFICATION. PLEASE POST THE CERTIFICATE ON THE WALL ONCE IT IS RECEIVED.

THE FACILITY RECEIVED A COMPLAINT FOR AN INDIVIDUAL BECOMING SICK AFTER EATING AT THIS LOCATION.

UPON INSPECTION, THE PIC STATED THAT NO COMPLAINTS HAD BEEN RECEIVED THROUGH THEIR COMPLAINT SYSTEM FOR INDIVIDUALS BECOMING ILL FROM CONSUMING FOOD AT THIS LOCATION. THE FACILITY DOES NOT COOK OR HANDLE RAW ANIMAL PRODUCTS. ALL FOOD ITEMS ARE RECEIVED FROM AN APPROVED SOURCE AND ANIMAL PRODUCTS ARE REHEATED. THE MANAGER WAS KNOWLEDGEABLE ABOUT COLD HOLDING, HOT HOLDING, AND REHEATING TEMPERATURES AS WELL AS A DATEMARKING PROCESS. ALL TEMPERATURES TAKEN DURING THE INSPECTION WERE SATISFACTORY. A FULL INSPECTION OF THE FACILITY WAS CONDUCTED ON 5/5/21.

THE FACILITY RECEIVED TWO VIOLATIONS. THE FACILITY DOES NOT HAVE AN INDIVIDUAL WITH A OHIO MANAGERS CERTIFICATION, ONLY A TACO BELL SERVSAFE CERTIFICATE WERE AVAILABLE TO BE VIEWED. THE FACILITY WILL BE EMAILED AN APPLICATION FOR RECIPROCITY IN ORDER TO OBTAIN THIS CERTIFICATION. IN ADDITION, UPON DISCUSSION WITH THE PIC, NO INDIVIDUALS WHO ARE SHIFT LEADS OR ASSISTANT MANAGERS HAVE A LEVEL ONE (PIC) CERTIFICATION. THE PIC WAS INSTRUCTED THAT ALL INDIVIDUALS WHO OPERATE THE FACILITY AND ARE IN CHARGE DURING SHIFTS MUST HAVE THEIR LEVEL ONE CERTIFICATION IF THE INDIVIDUAL WITH THEIR OHIO MANAGERS CERTIFICATION IS NOT ON THE PREMISES. THE FACILITY ALSO HAS AN INSUFFICENT EMPLOYEE HEALTH POLICY WITH ONLY 5 REPORTABLE FOODBORNE ILLNESSES. THE FACILITY WAS EMAILED AN EMPLOYEE HEALTH POLICY TO USE AS THEIR OWN.
