Cutting blocks or boards cannot be effectively cleaned and sanitized, they have deep cuts and are discolored.

Non-food contact surfaces of equipment are unclean. The racks in the produce walk in refrigerator are soiled. The can opener holder is soiled. The inside bottom of the keg refrigerator bottom is soiled.

Facility not maintained clean. There is a buildup of dust on the insulation that is loose over the three compartment sink in the kitchen. The is a buildup of dust on the walls and ceiling in the beer walk in refrigerator and a build-up of dust on the fan grate in the produce walk in refrigerator. The doors to the walk in refrigerators and jambs are soiled.

Physical facilities not maintained in good repair. The insulation of the ventilation ducts in the kitchen is loose. The wall behind the mixer is damaged and the coving is missing.

Insufficient air gap between the flood rim and the water supply inlet. The prep sink drain extends below the flood rim of the mop sink. There needs to be a physical air gap between the drain pipe and the flood rim of the mop sink.
