Critical Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. OBSERVED: EMPLOYEES WERE NOT INFORMED OF THEIR RESPONSIBILITY TO REPORT THEIR ILLNESSES. PIC ALREADY HAD THE LITERATURE.

Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. OBSERVED: PIC DID NOT HAVE A PROCEDURE TO ADDRESS VOMIT/FECAL EVENTS. PIC ALREADY HAD LITERATURE. I EXPLAINED THE LITERATURE.

The physical facilities are not being maintained in good repair. OBSERVED BROKEN FLOOR BOARD IN REAR OF STORE.

3717-1-06.4(F) / Drying mops.
Observed mops being dried using an unapproved method. OBSERVED MOP STANDING IN DIRTY WATER.
After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
