3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. OBSERVED A WORKING CONTAINER (SPRAY BOTTLE) WITHOUT THE NAME OF THE CHEMICAL ON THE BOTTLE.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.THE BOTTLE WAS DISCARDED DURING THE TIME OF INSPECTION. 3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN APPROVED FOOD SAFETY TRAINING COURSE, OBTAIN THE COURSE CERTIFICATE UPON COURSE COMPLETION, THEN CONTACT THE OHIO DEPARTMENT OF HEALTH FOR ADDITIONAL INFORMATION TO OBTAIN THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED BROKEN FLOOR TILE IN THE BAKERY DEPARTMENT NEAR THE WALK IN COOLER AND FREEZER.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. REPAIR THE FLOORING SO THAT IT IS SMOOTH AND EASILY CLEANABLE AT ALL TIMES.
3717-1-06.2(I)(1) / Lighting - intensity (10)
Repeat The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. OBSERVED THAT THE LIGHT INTENSITY WAS LESS THAN 10 FOOT CANDLES INSIDE OF THE BAKERY DEPARTMENT'S WALK IN FREEZER.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. INCREASE THE LIGHT ITENSITY TO AT LEAST 10 FOOT CANDLES INSIDE OF THE BAKERY DEPARTMENT'S WALK IN FREEZER TO AVOID A POSSIBLE HAZARD.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT AND FOOD DEBRIS ON THE BAKERY DEPARTMENT'S WALLS AND FLOORS.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. THOROUGHLY CLEAN FLOORS, WALLS THROUGHOUT THE FACILITY TO PREVENT POSSIBLE PEST INFESTATION AND POSSIBLE HARBORAGE.
