3717-1-02.1(A) / Employee health: Reporting by food employees
Critical License holder did not require employee(s) to report symptoms, an illness diagnosed by a health care provider, or high risk situations as specified by law. OBSERVED NO EMPLOYEE HEALTH POLICY.
To prevent the transmission of foodborne illnesses, the license holder shall require food employees and conditional employees to report to the person in charge information about their health as it relates to diseases that are transmissible through food. PIC WAS GIVEN A COPY OF AN EMPLOYEE HEALTH POLICY TO USE AS THEIR OWN. EVERY EMPLOYEE SHOULD READ AND SIGN THE EMPLOYEE HEALTH POLICY. Correct By: 07-Feb-2018
3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection TCS foods were not being held at the proper temperature. OBSERVED HOT DOGS AND ITALIAN SAUSAGE IN THE DELI DISPLAY CASE THAT RANGED FROM 45-50 DEGREES F. THE COOLER WAS 41 DEGREES F OR BELOW BUT THE PRODUCT WAS STACKED TIGHT IN THE COOLER REDUCING AIR FLOW TO THE PRODUCTS IN THE FRONT.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. ALL HOT DOGS AND ITALIAN SAUSAGE TEMPED AT 45-50 DEGREES F WAS DISCARDED. SEE VOLUNTARY DESTRUCTION FORM.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. OBSERVED NO AIR GAP FOR THE 3 COMPARTMENT SINK IN THE MEAT DEPARTMENT.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. 3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED NO EMPLOYEE WITH A LEVEL TWO CERTIFICATION IN FOOD SAFETY.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. PIC HAS SERVSAFE AND WAS GIVEN AN APPLICATION FOR RECIPROCITY.
3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items. OBSERVED FOOD IN BOXES ON THE FLOOR OF THE WALK IN FREEZER
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. PLACE ALL FOOD IN THE WALK IN FREEZER AT LEAST 6 INCHES ABOVE THE FLOOR.
3717-1-06.2(I)(1) / Lighting - intensity (10)
The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. OBSERVED A LIGHT OUT IN THE WALK IN FREEZER RESULTING IN LESS THAN TEN FOOT CANDLES. OBSERVED THE PRODUCE COOLER HAD A LIGHT INTENSITY OF 0-1.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
