Received a complaint stating: "The carrots in the chicken noodle soup were dirty. Found mice droppings in the salt shaker. One toilet in the women's rest room does not work and the other one is leaking".
Sanitarian unobserved dirty carrots in today's chicken noodle soup. Observed a mouse which quickly disappeared into a recent opening located near the rear of the Food Service operation. General Pest control will be notified whom a recent application occurred on 4-6-2022. Under the bread storage racks mice droppings are present and need to be removed. Note: the women's rest room has received maintenance and is working and no longer leaking.
