3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed stained, missing, broken ceiling tiles in back food prep. areas.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair or replace all old, stained, broken, missing ceiling tiles throughout Market where necessary.

Person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events..
Person in charge shall ensure that the FSO or RFE have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces.
3717-1-05.4(D)/Receptacles.
Observed the use of unapproved receptacles and waste handling units for refuse, recyclables, or returnables. Observed dirty waste cans with an accumulation of waste and food debris in customer dining area.
Receptacles and waste handling units for refuse, recyclables, or returnables and for use with materials containing food residue shall be durable, cleanable, insect and rodent-resistant, leak proof, and nonabsorbent. Wash and thoroughly clean dirty waste cans in customer dining area.
3717-1-06.1(M)/Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Large back delivery doors remain open throughout Day
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. Keep delivery doors at back of Supermarket closed when not receiving deliveries.
