Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. The milk had a date mark of 10/1/18. TCS food, milk, when the container is open, must be date marked. It must be discarded 6 calendar days after it is opened.

Facility does not have a person with level two certification in food protection.

No written procedures for vomiting or diarrheal events.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Food thermometer not readily accessible. There is no thermometer in the upright refrigerator.

3717-1-04.2(G)(2) / Food temperature measuring devices - measuring the temperature of thin foods.
No thin probe thermometer available. The probe thermometer goes from 50F-500F. Need a thermometer that checks the temperature of the cold foods.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. There is storage shelf that is 2" above the floor. The shelf must be 6" above the floor.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. The ceiling tiles are not a smooth and easily cleanable. When they become damaged they must be replaced with an approved type.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. The wood above the three compartment sink is not sealed. Seal the wood.
