3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. OBSERVED A BROWN SUBSTANCE BUILD-UP INSIDE OF THE ICE MACHINE.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Correct By: 05-Aug-2015

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Observed employee eating, drinking, or using tobacco in non-designated area. OBSERVED CIGARETTE ASHES IN THE SINK AND A LIGHTER ON THE CRATE IN THE UNISEX RESTROOM.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
A food thermometer was not readily accessible. OBSERVED THAT THERE WAS NO THERMOMETER IN THE FACILITY.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
There is no test kit available for measuring the concentration of the sanitizer.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED MISSING AND BROKEN TILE IN THE FACILITY.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.2(B) / Handwashing cleanser - availability.
Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink(s). OBSERVED THAT THERE WAS NO HANDWASHING SOAP AT THE HANDWASHING SINK NEXT TO THE THREE COMPARTMENT SINK NOR THE RESTROOM SINK.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Observed no towels or hand drying device at the handwashing sink(s). OBSERVED NO HAND TOWELS AT THE HANDWASHING SINK NEXT TO THE THREE COMPARTMENT PREP SINK NOR THE RESTROOM SINK.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT ON THE KITCHEN FLOOR.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED DEBRIS, LITTER AND CRUSHED BUILDING MATERIAL (CONCRETE) AROUND THE DUMPSTER.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. THE DEBRIS AND LITTER WERE SWEPT AT THE TIME OF INSPECTION, BUT THE CONCRETE PIECES ARE STILL PRESENT BEHIND THE DUMPSTER.
