Critical Repeat Corrected During Inspection PIC unable to demonstrate knowledge by having no critical violations. OBSERVED A CRITICAL VIOLATION IN THE FACILITY AT THE TIME OF THE INSPECTION. IN ORDER TO PREVENT CRITICAL VIOLATIONS, THE PIC SHALL MAINTAIN MANAGERIAL CONTROL AT ALL TIMES. THE PIC CORRECTED THE CRITICAL VIOLATION AT THE TIME OF THE INSPECTION.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. OBSERVED POTATO SALAD AND COLESLAW PAST THEIR DATEMARKS. IN ORDER TO PREVENT THE GROWTH OF PATHOGENS, ALL READY TO TCS FOODS MUST BE DISCARDED BY THEIR DATEMARK. PIC VOLUNTARILY DISCARDED ITEMS.

Repeat Facility does not have an employee with manager certification in food protection. OBSERVED NO PERSON WITH AN OHIO MANAGERS CERTIFICATION IN FOOD PROTECTION.

Repeat Inadequate number of handwashing sinks. OBSERVED NO HANDWASHING SINK IN THE KITCHEN. THE ONLY AVAILABLE HANDSINK IS IN THE BATHROOM IN THE HALL.

Handwashing sink water below 100°F. OBSERVED THE HANDWASHING SINK IN THE BATHROOM WAS ONLY REACHING 55 DEGREES F.

3717-1-04.1(Y) / Temperature measuring devices.
Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. OBSERVED NO TEMP GAUGE IN THE PREP COOLER OR TWO DOOR COOLER IN THE ADJACENT ROOM.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. OBSERVED THE BOTTOM OF THE ONE DOOR FREEZER AND THE BOTTOM OF THE TWO DOOR COOLER IN THE ADJACENT ROOM ARE SOILED WITH FOOD DEBRIS.

Repeat Physical facilities not maintained in good repair. OBSERVED THE ROOF IS LEAKING IN MULTIPLE LOCATIONS THROUGHOUT THE FACILITY. OBSERVED WATER DAMAGED CEILING TILES IN THE FACILITY. PLEASE REPAIR ROOF AND CEILING TILES.

III - Preventing Contamination by Hands: Observed a handwashing sink without water at the required temperature. OBSERVED THE HANDWASHING SINK IN THE BATHROOM WAS ONLY ABLE TO REACH 55 DEGREES F. IN ORDER TO PROPERLY WASH HANDS, WARM WATER TO 100 DEGREES MUST BE SUPPLIED TO THE SINKS. PLEASE INCREASE WATER TEMPERATURE FOR THE BATHROOM SINKS.

VI - TCS Food: Ready-to-eat, TCS food that had been date marked was not properly discarded when required. OBSERVED COLESLAW AND POTATO SALAD WERE PAST THEIR DATEMARKS. IN ORDER TO PREVENT THE GROWTH OF PATHOGENS, ALL READY TO EAT TCS FOODS SHOULD BE DISCARDED BY THEIR DATEMARK. THE PIC VOLUNTARILY DISCARDED THE SALADS.

X - P - Chemical: Toxic materials are properly identified and stored. OBSERVED TOXIC CHEMICALS STORED IN THEIR DESIGNATED AREA. IN ORDER TO PREVENT CONTAMINATION, ALL TOXIC CHEMICALS SHOULD BE STORED IN A DESIGNATED AREA AWAY FROM FOOD, DISHWARE, AND UTENSILS.
