3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED NO EMPLOYEE WITH A LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-03.1(A)(3) / Sources - packaged food labeling
Observed food packages that are not properly labeled. OBSERVED COTTAGE FOOD PRODUCTS THAT WERE BEING SOLD IN THE STORE THAT HAD INCORRECT LABELING. THE PIC WAS TOLD TO HAVE THE INDIVIDUAL WHO SUPPLIES THE BAKERY ITEMS TO RELABEL THE ITEMS BEFORE SELLING IN THE STORE.
To properly inform consumers, packaged food shall be labeled as specified in law. ENSURE THAT ALL COTTAGE FOOD PRODUCTS SOLD IN THE STORE ARE LABELED CORRECTLY. THE PIC WAS GIVEN A CORRECT LABELING HANDOUT THAT DETAILS THE NECESSARY ITEMS ON A LABEL.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. OBSERVED A BROKEN SLUSHY MACHINE.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. REPAIR OR REMOVE THE SLUSHY MACHINE
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWASHING SIGNS IN THE BATHROOMS.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. OBSERVED A WALL NEAR THE FRONT OF THE STORE THAT NEED TO BE REPAIRED. IT WAS DAMAGED IN AN ATTEMPTED BREAK IN TO THE FACILITY.
The physical facilities shall be maintained in good repair. REPAIR THE WALL IN THE FRONT OF THE STORE.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. OBSERVED WATER DRIPPING FROM AROUND THE FAN IN THE WALK IN COOLER INTO A CONTAINER.
The physical facilities shall be maintained in good repair. REPAIR THE WALK IN COOLER.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Corrected During Inspection Observed a build-up of dirt and debris. OBSERVED DIRTY FAN SHIELDS IN THE WALK IN COOLER.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. AN EMPLOYEE STARTED CLEANING THE FAN SHIELDS WHILE THE INSPECTION WAS BEING CONDUCTED. MAINTAIN THE FAN SHIELDS SO THEY ARE NOT DUSTY.
