[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN APPROVED LEVEL TWO FOOD SAFETY TRAINING COURSE, OBTAIN THE CERTIFICATE UPON COURSE COMPLETION, AND CONTACT THE OHIO DEPARTMENT OF HEALTH FOR ADDITIONAL INSTRUCTIONS TO RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED THAT THE FIVE DOOR REACH IN FREEZER (REAR OF FACILITY) HAS AN AMBIENT TEMPERATURE OF 56 DEGREES FAHRENHEIT.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. IMMEDIATELY DISCONTINUE USE OF THIS UNIT UNTIL ALL NECESSARY ADJUSTMENTS/REPAIRS HAS BEEN MADE AND THE CLEVELAND HEALTH DEPARTMENT HAS MADE THE APPROVAL FOR RE-USE OF THE REACH IN FREEZER.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Utensils, thermometers, and/or pressure guages were not kept in good repair or calibration. OBSERVED THAT THE EQUIPMENT THERMOMETER FOR THE FIVE DOOR REACH IN FREEZER IS NOT WORKING PROPERLY WORKING (THE TEMPERATURE COMES ON, THEN IT GOES OFF).
(1) Utensils shall be maintained in a state of good repair or shall be discarded; (2) Food temperature measuring devices shall be calibrated as necessary to esnure their accuracy; (3) Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. PROVIDE A NEW EQUIPMENT THERMOMETER FOR THE FIVE DOOR REACH IN FREEZER AND CHECK IT REGULARLY TO VERIFY THAT THE UNIT STAYS AT OR BELOW ZERO DEGREES FAHRENHEIT AT ALL TIMES.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. OBSERVED A BUILD-UP OF DIRT ON THE WALLS, RACKS AND FLOORS OF THE WALK IN COOLER.
Nonfood-contact surfaces of equipment shall be kept clean. THOROUGHLY CLEAN THE WALK IN COOLER IN ORDER TO PREVENT POSSIBLE PEST INFESTATION.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED THAT THERE IS NO HANDWASHING SIGN AT THE UNISEX RESTROOM.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. PROVIDE A HANDWASHING SIGN AT THE UNISEX RESTROOM.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. OBSERVED A DAMAGED CEILING INSIDE OF THE KITCHEN.
The physical facilities shall be maintained in good repair. REPAIR THE DAMAGED KITCHEN CEILING.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED AN ICE MACHINE, LAWN MOWER, AND OTHER ITEMS IN THE KITCHEN.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE ALL EQUIPMENT THAT IS NOT IN USE OR NON-FUNCTIONING FROM THE ENTIRE FACILITY TO PREVENT POSSIBLE PEST INFESTATION AND POSSIBLE HARBORAGE.
