3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. OBSERVED UNCOVERED FOOD IN THE COOLER.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. ALL FOODS WERE COVERED AT THE TIME OF INSPECTION.
3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of live insects, rodents, and other pests. OBSERVED PRESENCE OF FLIES IN THE FACILITY (HOT FOODS AREA, DELI AND IN THE KITCHEN COOLER).
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. 3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed improper storage of food items. OBSERVED FOODS (EGGS AND VEGETABLES) NOT STORED AT LEAST SIX INCHES OFF FROM THE GROUND.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. THE FOODS WERE PLACED AT LEAST SIX INCHES OFF FROM THE GROUND AT THE TIME OF INSPECTION.
3717-1-03.5(C)(1) / Food labels - packaged food
A food packaged in the facility did not contain a label. OBSERVED SEVERAL ITEMS ON DISPLAY IN THE FACILITY WITHOUT A LABEL: PICKLED CARROTS, PICO DE GALLO, GUACAMOLE, PICKLED CACTUS, GREEN SALSA AND RED SALSA.
Foods shall be labeled as specified in this rule. THE FACILITY MUST DISCONTINUE PRODUCTION AND SALE OF THE ABOVE MENTIONED PRODUCTS UNTIL FURTHER NOTICE. Correct By: 04-Oct-2015
3701–21-27 OAC / Embargo of Food
Corrected During Inspection Observed food in a FSO that had been adulterated, contaminated, misbranded, or otherwise unsafe, and was embargoed. OBSERVED EXPIRED EMPANADA DOUGH SHELLS SITTING ON THE KITCHEN COOLER RACK.
When a licensor finds food that is adulterated or so misbranded as to be dangerous or fraudulent, the licensor shall affix to the item a tag giving notice that the food has been embargoed. THE ITEMS WERE REMOVED AND DISCARDED AT THE TIME OF INSPECTION.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED LOOSE CEILING TILE DIRECTLY OVER THE COOKING EQUIPMENT.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF DIRT AND DEBRIS UNDERNEATH AND BEHIND THE COOKING EQUIPMENT.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

REPLACE THE EXPIRED (07-15-2011) QUATERNARY TEST KIT WITH A CURRENT, CHLORINE BASED KIT AS LONG AS CHLORINE WILL BE USED AS THE FACILITY'S SANITIZER. PROVIDE A WAY TO CONTROL THE INSECTS (FLIES) IN THE FACILITY BY 10-08-2015.
