IN ORDER TO PREVENT CRITICAL VIOLATIONS, THE PIC MUST MAINTAIN MANAGERIAL CONTROL OVER THE FACILITY AT ALL TIMES. PIC CORRECTED ALL CRITICAL VIOLATIONS AT THE TIME OF THE INSPECTION.

Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. OBSERVED TWO SOILED POP NOZZLES AND ONE SOILED TEA NOZZLE. IN ORDER TO PREVENT CONTAMINATION, ALL FOOD CONTACT SURFACES SHOULD BE MAINTAINED CLEAN TO SIGHT AND TOUCH. PIC WASHED AND SANITIZED NOZZLES AT THE TIME OF THE INSPECTION.

Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. OBSERVED QUAT SANITIZER AT 0-50PPM AT THE THREE COMPARTMENT SINK. IN ORDER TO PROPERLY SANITIZE DISHES AND UTENSILS, THE QUAT SANITIZER SHOULD BE BETWEEN 200-400PPM. PIC CORRECTED THE QUAT SANITIZER AT THE TIME OF THE INSPECTION.

3701-21-25(I) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection. OBSERVED NO PERSON WITH LEVEL 1 CERTIFICATION. THE PERSON WITH THE OHIO MANAGERS CERTIFICATION WAS NOT ON SITE.
