Critical Corrected During Inspection Chlorine sanitizing solution at incorrect temperature and/or concentration. OBSERVED THE 3 COMPARTMENT SINK HAD CHLORINE AT A CONCENTRATION OF 200PPM. THE DISHWASHING EMPLOYEE WAS SHOWN WHAT CONCENTRATION OF CHLORINE IS APPROPRIATE AND THE SOLUTION WAS CHANGED TO 100 PPM.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. OBSERVED RICE IN THE COOLER DATED 6/7 WITH MOLD ON IT. PIC DISCARDED THE RICE AT THE TIME OF THE INSPECTION.

Corrected During Inspection [51] Improper storage of cleaned equipment, utensils, and laundered linens. OBSERVED CLEAN UTENSILS STORED IN CONTAINERS WITH FOOD DEBRIS AT THE BOTTOM. OBSERVED THE ICE SCOOPERS STORED IN A SOILED CONTAINER.

Cutting blocks or boards cannot be effectively cleaned and sanitized. OBSERVED THE CUTTING BOARD IN THE KITCHEN WAS SCORED AND STAINED AND NO LONGER ABLE TO EFFECTIVELY SANTIZED.

Facility not maintained clean. OBSERVED THE FLOOR WAS SOILED UNDER THE PREP AREA NEAR THE ICE MACHINE.

Physical facilities not maintained in good repair. OBSERVED THE CEILING TILE ABOVE THE WALK IN COOLER WAS DAMAGED.

Light intensity less than fifty foot candles in required areas. OBSERVED A BULB OUT UNDER THE HOOD OVER THE GRILL RESULTING IN LESS THAN 50 FOOT LIGHT CANDLES OF LIGHT.

I - P - Employee Health: The operation had an employee health policy on file. OBSERVED THE FACILITY HAD EMPLOYEE HEALTH POLICIES ON FILE. ALL EMPLOYEES MUST READ AND SIGN AN EMPLOYEE HEALTH POLICY WHICH CONTAINS THE COMMON FOODBORNE ILLNESS SYMPTOMS, THE 13 REPORTABLE DISEASES, AND A STATEMENT OF ACKNOWLEDGEMENT. ALL POLICIES MUST BE KEPT ON FILE AT ALL TIMES IN THE FACILITY.

VI - P - TCS Food: Observed hot foods being held at 135°F or above; cold foods being held at 41°F or below. OBSERVED COLD FOODS BEING HELD AT 41 DEGREES F AND HOT FOODS BEING HOT HELD AT 135 F OR ABOVE. TCS FOODS SHOULD NEVER ENTER THE TEMPERATURE DANGER ZONE WHICH EXISTS BETWEEN 41 DEGREES F AND 135 DEGREES F.

VI - TCS Food: Ready-to-eat, TCS food that had been date marked was not properly discarded when required. OBSERVED RICE DATED 6/7 WAS NOT DISCARDED AND HAD MOLD GROWTH ON IT. ALL DATEMARKED FOODS MUST BE DISCARDED WITHIN 7 DAYS OF PREPARATION.

VI - TCS Food: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. OBSERVED FOOD IN THE PREP COOLER, A REACH IN COOLER, AND THE WALK IN COOLER THAT WAS NOT DATEMARKED. ALL REFRIGERATED READY TO EAT TCS FOODS MUST BE DATEMARKED AND DISCARDED WITHIN 7 DAYS OF PREPARATION. THE DAY OF PREPARATION COUNTS AS THE 1ST DAY.
