Critical Written procedures and plans are not maintained and implemented. There is no vomit/diarrhea clean up kit per the policy.

Critical Repeat Corrected During Inspection Improper bare-hand contact with RTE foods. The bartender was slicing a lime with their bare hands. Discarded lime.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. There was a buildup of soil on the shield inside the ice machine. Empty, wash, rinse and sanitize before making ice.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. The precooked fries were 43F and the hot dogs were 43F in the basement refrigerator. Items were discarded.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. The chile was over 24 hours old and was not dated, item discarded. Some items had a production date and others had a discard date. It must be consistent what dating system that is used in the operation. If the dating varies the items past the date will be discarded.

Critical Corrected During Inspection Improper storage of poisonous or toxic materials. There was a case of pine-sol that was stored over film wrap and beverage stirrers in the basement.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have a person with level two certification in food protection.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. The flour breadings was stored on a shelf over night without a cover. This allows for contamination. The beadings were discarded.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. The ice scoops in the basement for the ice machine are stored in a soiled pan.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Corrected During Inspection Single service items are stored under pine sol in the basement.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. There are cutting boards with deep cuts and discolored.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. The bottom of the upright refrigerator in the basement has a unknown liquid in the bottom. Clean on a frequent basis.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. There is the smell of sewage in the kitchen and the lid to the grease trap is not sealed. Seal lid.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. There are open and exposed rafters over the ice machine in the basement. Install a smooth sealed surface over the ice machine in the basement.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. The floor is dirty on the cookline under equipment and shelving unit. The steps are soiled going to the basement and in the basement. Clean on a frequent basis.

FSO did not have a person in charge per shift with level one certification in food protection.
