3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical The person in charge was unable to demonstrate proper knowledge of food safety and prevention. OBSERVED CRITICAL VIOLATIONS DURING THE INSPECTION.
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection. IN ORDER TO AVOID CRITICAL VIOLATIONS, THE PIC SHALL MAINTAIN CONTROL AND OVERSIGHT OVER THE FACILITY AT ALL TIMES. Correct By: 23-Feb-2018
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked. OBSERVED MAC AND CHEESE AND PASTA WITH NO DATE MARKS.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. ALL PREPARED FOOD SHOULD HAVE A DATE MARK IF KEPT UNTIL THE NEXT DAY. THE FOOD SHOULD THEN BE USED WITHIN 7 DAYS OF THAT DATE MARK. THE DAY OF PREPARATION IS COUNTED AS THE FIRST OF THE 7 DAYS. Correct By: 23-Feb-2018
3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of live insects, rodents, and other pests. OBSERVED MOUSE FECES IN THE WALK IN COOLER, BEHIND A 3 DOOR FREEZER AND BEHIND A RACK OF DISHES.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. PIC HAS HAD THE EXTERMINATOR OUT RECENTLY. PIC WAS TOLD TO CLEAN THE WALK IN COOLER AND BEHIND THE FREEZER AND STORAGE RACK AND MONITOR THE AREAS FOR FURTHER EVIDENCE OF A PEST PROBLEM AS WELL AS INCREASING PEST CONTROL VISITS. 3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED NO EMPLOYEE WITH A LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
A food thermometer was not readily accessible. OBSERVED NO THERMOMETER FOR TAKING THE TEMPERATURES OF BOTH HOT AND COLD FOODS. ONLY A MEAT THERMOMETER WAS AVAILABLE.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED THE SMALL PREP COOLER WAS HOLDING SOME WATER ON THE BOTTOM SHELF.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. REPAIR THE SMALL PREP COOLER SO THAT IS NOT HOLDING WATER.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. OBSERVED NO LIGHT COVER ON A LIGHT LOCATED UNDER THE HOOD.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED MOUSE FECES ON THE FLOOR BEHIND THE FREEZER AND A STORAGE RACK.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. CLEAN THOROUGHLY IN ORDER TO MONITOR THE PEST SITUATION.
