Repeat Non-food contact surfaces of equipment are unclean. OBSERVED: MICE DROPPINGS THROUGHOUT THE FACILITY, FEEDING ON FOOD PACKAGES, CARDBOARD BOXES, LABELS ON CANS OF FOOD, AND URINE ON TOP OF CAN GOODS.

11/7/18 NOTE: DISTRICT MANAGER HAS INCREASED PEST CONTROL TO TWICE PER/WEEK, AND INCREASE DUMPSTER SERVICE TO DAILY FOR THE NEXT 10 DAYS. TRAPS ARE DOWN AND THE KICK-PLATES ARE BEING REMOVED AND CLEANING HAS BEGUN TO REMOVE THE DIRTY, DEBRIS, WASTE THAT IS THERE. THE MANAGER OF THE MALL HAS INCREASE PEST CONTROL TO ONCE PER/WEEK.
NON-COMPLIANCE OF VIOLATIONS HAS PERSISTED SINCE MY FIRST VISIT 10/25/18. A RISK MANAGEMENT PLAN IS RECOMMENDED.
