Date the last shellstock was sold or served was not recorded on the label. The date was not recorded on mussel tags from November and December. Document the date the last shellfish for that lot is sold.

Food not properly protected from contamination by separation, packaging, and segregation. There are cans of paint stored over sunny delight juice in the rear storage area.

Equipment food-contact surfaces or utensils are unclean. Inside the box of the ice machine in the meat and seafood area the side and chute were soiled. Empty, wash, rinse and sanitize before making ice.

Refrigerated, ready-to-eat, TCS foods not properly date marked. The deli department is using production date and the bakery area is using discard date. Develop a system the whole store will follow.

Improper storage of poisonous or toxic materials. Toxic items were stored over canning jars and pectin. Move jars and pectin.

Fresh juices packaged without proper labeling.Fresh juice is made on site without a warning statement. Warning: This product has not been pasteurized and, therefore, may contain harmful bacteria that can cause serious illness in children, the elderly, and persons with weakened immune systems. This warning must be on all of the bottles. Discontinue bottling until the warning is on the bottle also.

Working food containers not properly labeled. Working containers in the sushi area were not labeled.

Bulk food for customer self-service not properly labeled. Bulk containers are missing labels on 3 containers.

Raw fruit and/or vegetables not washed or washed improperly. The oranges used in the juice making machine are not all washed before being used, only the ones that appear soiled. All oranges used in the making of the juice on site must be washed to remove soil.

Non-durable equipment observed. The front cover on the orange juice maker is cracked. Replace.

Non-food contact surfaces of equipment are unclean. In the bakery area the topping cabinet door and bottom are soiled. Speed rack in the bakery is soiled.

Facility not maintained clean. The dairy walkin cooler has a buildup o the fan grates and the ceiling. The floor is soiled under equipment in the bakery and hot foods area.
