Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Raw eggs were stored over butter in the walk in refrigerator.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. The opened can of white beans that dated 12/15 was discarded.

Critical Repeat Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. The opened canned white beans were dated 12/15.

Critical Repeat Consumer is not properly advised of the risk of consuming raw or undercooked animal foods. The advisory is not asterisked and the menu items that the advisory applies to are not asterisked.

3717-1-07.1(A) / Poisonous or toxic materials: Storage: separation.
Critical Repeat Corrected During Inspection Improper storage of poisonous or toxic materials. Turbo chef cleaner was stored over single service items in the storage area.

3717-1-04.4(J) / Mechanical warewashing equipment - wash solution temperature.
Critical Corrected During Inspection Improper temperature of wash solution in mechanical warewasher. The wash temperature for the dishwasher was 113F after 7 cycles. Use three compartment sink until repaired.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have a person with level two certification in food protection.

3717-1-03.5(C)(2) / Food labels - packaged food
Corrected During Inspection Label on a food packaged in-house did not contain all required information. The salads prepared in house did not list the subingredients for the chicken and the cheese. The allergens were not declared on the label. There was also no statement of responsibility. Items were removed from display until the corrected labels are used.

Repeat No sanitizer test kit available. No QUAT test strips were available.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. The can opener holder was soiled.

3717-1-06.1(F) / Wall and ceiling - coverings and coatings.
Unapproved materials covering the walls and/or ceiling. Damaged ceiling tiles in the kitchen were replaced with tiles that are not approved. Replace tiles with an approved type.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. The drywall is not sealed above the three compartment sink. Seal dry wall.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Corrected During Inspection Light intensity less than fifty foot candles in required areas. Over the three compartment sink was 23 foot candles and over the prep tale there was only 18 foot candles. Increase the light levels.

3701-21-25(I) / Level one certification in food protection
FSO did not have a person in charge per shift with level one certification in food protection.
