Critical PIC unable to demonstrate knowledge by having no critical violations. THE PERSON IN CHARGE DID NOT COMPLY WITH THE CODE BY HAVING CRITICAL VIOLATIONS DURING THE CURRENT INSPECTION. TO EXERCISE ACTIVE MANAGERIAL CONTROL OVER THE FOOD SERVICE OPERATION, THE PIC SHALL COMPLY WITH THE CODE BY HAVING NO CRITICAL VIOLATIONS DURING THE CURRENT INSPECTION. THE SANITARIAN EDUCATED THE PIC ON WAYS TO PREVENT CRITICAL VIOLATIONS DURING SUBSEQUENT INSPECTIONS.

OBSERVED FOOD EMPLOYEE MADE IMPROPER BAR - HAND CONTACT IN A FIVE GALLON POT OF COOKED SPAGHETTI DURING THE COOL DOWN PROCESS IN COOKING / DISHWASHING AREA. TO PREVENT CONTAMINATION OF RTE FOODS, FOOD EMPLOYEES MAY NOT HAVE CONTACT TO FOODS WITH THEIR BAR HANDS AND SHALL USE UTENSILS SUCH AS SPATULAS, TONGS, AND - SINGLE USE GLOVES. THE PERSON IN CHARGE VOLUNTARY DENATURED THE COOKED SPAGHETTI.

Critical Presence of live insects, rodents, and other pests. OBSERVED RODENT DROPPINGS AND TWO COCK ROACHES IN COOKING AND DISHWASHING AREA. TO PREVENT THE SPREAD OF DISEASES AND BACTERIA, THE PRESENCE OF RODENTS AND INSECTS SHALL BE CONTROLLED BY TRAPPING DEVICES OR OTHER MEANS OF PEST CONTROL. PIC HAD NOT MADE CORRECTIVE ACTIONS STATED FROM THE PEST CONTROL COMPANY.

CUSTOMER COMPLAINT DUE TO UNSANITARY CONDITIONS THAT CAUSED RODENT ACTIVITY. OBSERVED AN ACCUMULATION OF FOOD PARTICLES THROUGHOUT THE KITCHEN AREA. OBSERVED (PHOTO TAKEN) RODENT DROPPINGS IN FRONT OF STORAGE SHELVES IN STORAGE FACILITY. OBSERVED OTHER PEST ISSUES SUCH AS (PHOTOS TAKEN) (TWO) GERMAN COCK ROACHES CRAWLING BEHIND PREP SINK NEXT TO WARE WASHING MACHINE IN COOKING / DISHWASHING AREA. PEST CONTROL DOCUMENTS WERE PROVIDED FROM THE PERSON IN CHARGE, HOWEVER, DOCUMENT (11-3-21) WAS STATED BY THE PEST CONTROL COMPANY OF THE ACTIONS THAT WERE NEEDED TO CORRECT THESE ISSUES. THEREFORE, THE ACTIONS THAT NEEDED TO BE ADDRESSED, WERE NOT CORRECTED. OBSERVED FOOD EMPLOYEE HAD MADE BARE HAND CONTACT BY PLACING HIS HANDS (WITHOUT GLOVES) INSIDE THE FIVE GALLON POT OF COOKED SPAGHETTI WHILE IN PROCESS OF COOLING DOWN IN COOKING / DISHWASHING AREA. THE PERSON IN CHARGE VOLUNTARY DENATURED THE COOKED SPAGHETTI. TO CONCLUDE, THAT THERE WERE EVIDENCE OF PEST (RODENT) ISSUES, THEREFORE, CODE VIOLATIONS WILL BE ISSUED.
