TO EXERCISE MANAGERIAL CONTROL OVER THE FOOD SERVICE OPERATION, THE PIC SHALL COMPLY WITH THE CODE BY HAVING NO CRITICAL VIOLATIONS DURING THE CURRENT INSPECTION. AT THE TIME OF THE INSPECTION, THE SANITARIAN INSTRUCTED THE PIC REGARDING THE CORRECTIVE ACTIONS NEEDED TO ENSURE NO CRITICAL VIOLATIONS OCCUR ON SUBSEQUENT INSPECTIONS.

AT THE TIME OF THE INSPECTION, THE SANITARIAN PROVIDED THE PIC WITH INFORMATION AND DOCUMENTATION REGARDING THE MAJOR FOOD ALLERGENS.

OBSERVED THE PERSON IN CHARGE (PIC) DID NOT PROPERLY DEMONSTRATE CALIBRATION OF THE FOOD TEMPERATURE MEASURING DEVICE. TO ACCURATELY MEASURE THE TEMPERATURE OF TCS FOOD, FOOD TEMPERATURE MEASURING DEVICES SHALL BE PROPERLY CALIBRATED. AT THE TIME OF THE INSPECTION, THE SANITARIAN INSTRUCTED THE PIC REGARDING PROPER THERMOMETER CALIBRATION TECHNIQUES AND PROVIDED THE PIC WITH SUPPORTING DOCUMENTATION.

Critical Corrected During Inspection Food employee(s) did not wash hands when required. OBSERVED FOOD EMPLOYEE DONNING SINGLE-USE GLOVES WITHOUT WASHING HANDS WHEN CHANGING TASKS. TO PREVENT CONTAMINATION OF FOOD WITH MICROORGANISMS THAT CAN LEAD TO ILLNESS, FOOD EMPLOYEES SHALL CLEAN THEIR HANDS AND EXPOSED PORTIONS OF THEIR ARMS IMMEDIATELY BEFORE ENGAGING IN FOOD PREPARATION, BEFORE DONNING GLOVES TO INITIATE A TASK THAT INVOLVES WORKING WITH FOOD, AND DURING FOOD PREPARATION AS OFTEN AS NECESSARY TO REMOVE SOIL AND CONTAMINATION AND TO PREVENT CROSS CONTAMINATION WHEN CHANGING TASKS, AND AFTER ENGAGING IN ANY OTHER ACTIVITIES THAT CONTAMINATE THE HANDS. OBSERVED FOOD DEBRIS IN THE HANDWASHING SINK IN THE KITCHEN; SEVERAL ICE CREAM CONES WERE PLACED IN THE SINK BEFORE AND AFTER SANITARIAN ARRIVAL. TO PREVENT CROSS-CONTAMINATION WITH MICROORGANISMS THAT CAN LEAD TO FOODBORNE ILLNESS, THE HANDWASHING SINK SHALL BE USED SOLELY FOR HAND WASHING PURPOSES. AT THE TIME OF THE INSPECTION THE SANITARIAN INSTRUCTED THE PIC REGARDING THE PERMITTED AND THE PROHIBITED USES OF A HANDWASHING SINK.

TO PREVENT CONTAMINATION OF FOOD WITH POISONOUS AND TOXIC SUBSTANCES SUCH AS FURNITURE PAINT AND ROOM FRAGRANCE, POISONOUS AND TOXIC MATERIALS SHALL BE STORED IN SUCH A WAY SO THAT THEY CANNOT CONTAMINATE FOOD, EQUIPMENT, UTENSILS, LINENS, SINGLE-SERVICE ARTICLES, AND SINGLE-USE ARTICLES. TO PREVENT CONTAMINATION OF FOOD WITH POISONOUS AND TOXIC SUBSTANCES SUCH AS WINDEX AND CLEANER, POISONOUS AND TOXIC MATERIALS SHALL BE STORED IN SUCH A WAY SO THAT THEY CANNOT CONTAMINATE FOOD, EQUIPMENT, UTENSILS, LINENS, SINGLE-SERVICE ARTICLES, AND SINGLE-USE ARTICLES. THIS VIOLATION WAS NOT CORRECTED AT THE TIME OF THE INSPECTION.

TO PREVENT CONTAMINATION OF FOOD, EQUIPMENT, AND UTENSILS, AND MINIMIZE THE CHEMICAL HAZARD TO EMPLOYEES, ONLY INCIDENTAL USE PESTICIDES SUCH AS BEE AND WASP PESTICIDES ARE PERMITTED IN THE FOOD SERVICE OPERATION IN ORDER TO AVOID IMMEDIATE PERSONAL HARM. AT THE TIME OF THE INSPECTION THE SANITARIAN INSTRUCTED THE PIC REGARDING THE PERMITTED AND PROHIBITED USES OF PESTICIDES IN A FOOD SERVICE OPERATION.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Insufficient air gap between the flood rim and the water supply inlet. OBSERVED NO AIR GAP BETWEEN THE FAUCET SPOUT AND THE FLOOD RIM OF THE WAREWASHING SINK. TO PREVENT CONTAMINATION CAUSED BY BACKFLOW, THE AIR GAP BETWEEN THE WATER SUPPLY INLET AND THE FLOOD LEVEL RIM OF THE WAREWASHING SINK SHALL BE AT LEAST TWICE THE DIAMETER OF THE WATER SUPPLY INLET AND MAY NOT BE LESS THAN ONE INCH. AT THE TIME OF THE INSPECTION THE SANITARIAN INSTRUCTED THE PIC REGARDING THE AIR GAP REQUIREMENT FOR BACKFLOW PREVENTION.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. OBSERVED THE WALK-IN FREEZER CONDENSATE LINE IN THE BACK STORAGE ROOM WAS NOT PROPERLY AIR GAPPED. TO PREVENT CONTAMINATION CAUSED BY BACKFLOW, THE DRAIN ORIGINATING FROM EQUIPMENT IN WHICH FOOD, PORTABLE EQUIPMENT, OR UTENSILS ARE PLACED, SUCH AS A WALK-IN FREEZER, SHALL CONTAIN AN INDIRECT CONNECTION IN WHICH THE CONDENSATE LINE MUST BE AT LEAST ONE INCH ABOVE THE FLOOR DRAIN. THIS VIOLATION WAS NOT CORRECTED AT THE TIME OF THE INSPECTION.
