Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. THE PERSON IN CHARGE IS WORKING TOWARDS RE-TAKING THE EXAMINATION. ONCE WHEN THE FOOD SAFETY COURSE (FIFTEEN HOURS AND APPROVED VIA THE OHIO DEPARTMENT OF HEALTH [ODH]) CERTIFICATE HAS BEEN OBTAINED, COMPLETE THE RECIPROCITY APPLICATION FORM AND SEND THE COMPLETED FORM, A COPY OF THE COURSE CERTIFICATE, AND A COPY OF THE COURSE CURRICULUM (NOT A GENERIC AGENDA) TO ODH FOR REVIEW. UPON APPROVAL, YOU WILL RECEIVE THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION.
3717-1-06.2(I)(1) / Lighting - intensity (10)
The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. OBSERVE THAT THERE IS NO LIGHTING INSIDE OF THE DRY GOODS STORAGE AREA.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. MAKE NECESSARY ADJUSTMENTS/POSSIBLE REPAIRS TO INCREASE THE LIGHTING INSIDE OF THE DRY GOODS STORAGE AREA FOR FINDING ITEMS AND FOR CLEANING PURPOSES.
