Critical Repeat Observed the presence of live insects, rodents, and other pests. Observed rodent droppings in basement.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. This is a critical violation. All criticals must be complied within 24 hours. Call exterminator to remove flying pests. Correct by July 4, 2018.

Repeat Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed open door in back of Market with no air curtain to help prevent the entrance of flying pests.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Repeat Floors, walls, and/or ceilings were not smooth and easily cleanable. Observed broken floor in meat room around drain.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair or replace broken floor in meat room within thirty days. Correct by: 02-Aug.-2018. Correct By: 08-Aug-2018

Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Thoroughly clean and reorganize basement floors and coolers,( to remove old bagged and canned products to help remove clutter.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat The ventilation system was not being properly cleaned or was creating a public health hazard. Observed dirty ceiling vents throughout Market.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. Wash and thoroughly clean all greasy, dirty, ceiling air vents throughout Market where necessary.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. Observed broken front door at Supermarket entrance.

Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-04.2(G)(1)/Food temperature measuring devices - availability
A food thermometer was not readily accessible. There were no ambient thermometers inside refrigeration / freezer unit.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are dirty. Meat cutting table in meat cutting area is pitted with deep grooves and cuts which cannot be thoroughly cleaned.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Replace meat cutting table in meat cutting area within thirty days.
3717-1-04.5(D)/Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. Equipment non-food contact surfaces are dirty. Observed Dairy cooler with rust and built up debris.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-05.1(S)(1)/Plumbing system - maintained in good repair.
Eliminate stagnant water in basement.Example - trash can holding driupping sewer water and leaking onto floor, by floor drain. Correct pipe dripping on to wooden table and floor in basement.
3717-1-06.4(L)/Removing dead or trapped birds, insects, rodents, and other pests.
Observed the presence of dead pests or insects. Observed 'mosquito tape' in back, Bakery area with numerous dead, flying insects.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
3717-1-07(B)/Poisonous or toxic materials: Working containers - common name.
Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. Observed Clorox stored in clear bottle labeled glass cleaner.
To prevent health hazards, working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
