EGG CARTONS ARE NOT WATERPROOF/CLEANABLE CONTAINERS AND SHALL NOT BE STORED LESS THAN SIX INCHES FROM THE GROUND.

Observed an outer opening, window, or door without proper protection against the entry of insects and rodents.OBSERVED REAR DOOR WITHOUT DOOR SWEEPER FOR PROTECTION AGAINST THE ENTRY OF INSECTS/RODENTS.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.TO CORRECT, INSTALL DOOR SWEEPER AT THE BOTTOM OF REAR DOOR SO THAT THE DOOR IS CLOSED AND TIGHT-FITTING.
3717-1-06.2(L) / Service sinks - availability.
Observed no service sink or curbed cleaning facility.OBSERVED A UTILITY SINK WITH DIRT/ DEBRIS BUILD UP BEING USED TO CLEAN FOOD EQUIPMENT AND UTENSILS .OPERATION DID NOT HAVE A MOP SINK.
A service sink or curbed cleaning facility shall be provided as specified under paragraph (I) of rule 3717-1-05.1 of this code. TO PREVENT CONTAMINATION OF FOOD EQUPMENT, WASH, RINSE, AND SANITIZE UTLITY SINK BEFORE CLEANING THE COFFEE POT. ALSO, TO PREVENT STAGNANT SURFACE WATER WHICH CAN BE A BREEDING GROUND FOR PEST, THE PIC WAS ADVISED TO STOP DUMPING MOP WATER OUTSIDE.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. OBSERVED BROKEN GASKET SEALS ON ALL OF THE DOORS OF THE REAR REACH IN REFRIGERATION UNIT AND OBSERVED A BROKEN GASKET SEAL ON THE DOOR OF THE DAIRY WALK IN COOLER.
The physical facilities shall be maintained in good repair.REPLACE ALL DOOR GASKET SEALS ON THE REAR REFRIGERATION UNIT AND DAIRY WALK IN COOLER DOOR, TO ENSURE THAT PROPER TEMPERATURES ARE BEING MAINTAINED AT 41 DEGREES F OR BELOW.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris.OBSERVED DIRT/DEBRIS BUILD UP ON FOOD STORAGE SHELVES AND DEBRIS/TRASH ON THE FLOOR IN THE WALK IN COOLERS.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. CLEAN FOOD STORAGE SHELVES AND FLOORS ON A ROUTINE BASIS TO PREVENT BUILD UP OF DIRT/TRASH/DEBRIS. A WASTE RECEPTACLE IS RECOMMENDED TO BE STORED ON THE OUTSIDE OF THE WALKI-N COOLER TO ENSURE THAT TRASH/DEBRIS IS PROPERLY DISCARDED.

PEST CONTROL-BILLY BOMB SQUAD.PROVIDED PIC (PERSON IN CHARGE) WITH AN EMPLOYEE HEALTH POLICY, VOMIT/FECAL LEAN UP PROCEDURE, AND VARIOUS FOOD SAFETY HANDOUTS. FROZEN ICE CREAM FOOD ITEMS WERE REMOVED FROM SERVICE AND RETURNED TO VENDOR DUE TO IMPROPER STORAGE. SEE VOLUNTARY DESTRUCTION FORM.NOTE:ON APRIL 1, 2016 THERE WAS A FIRE IN THE FACILITY, RENOVATIONS WERE DONE, AND THE FIRE DEPARTMENT INSPECTED AND APPROVED THE FACILITY TO RE-OPEN FOR BUSINESS.
