3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food that had been date marked was not properly discarded when required. OBSERVED MULTIPLE FOOD CONTAINERS WITH DATE MARKING PAST THE 7 DAY RULE: MASHED POTAOES, VINGARETTE DRESSING, CREME FRESH, MUSTARD JUS.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if it: (a) exceeds the temperature or time specified in (G)(1) of this rule, except the time the food is frozen; (b) is in a container or package that does not bear a date or day; or (c) is marked with a date or day that exceeds the time and temperature combination specified in (G)(1) of this rule. 3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. OBSERVED ICE MACHINE WITH A MILDEW BUILD UP.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. TO PREVENT CONTAMINATION CLEAN ICE MACHINE AT A FREQUENCY TO PREVENT MILDEW BUILD UP. Correct By: 03-Dec-2017
3717-1-05.1(D) / Backflow prevention - air gap.
Critical Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. OBSERVED THE DRAIN PIPE FROM THE DISHWASHER DESCENDING INTO THE FLOOR DRAIN PREVENTING AN AIR GAP. ALSO OBSERVED DRAIN PIPE FROM ICE MACHINE BELOW FLOOD RIM ON DISCHARGE SINK PREVENTING AN AIR GAP.
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters). TO PREVENT CONTAMINATION, CUT PIPE TO PROVIDE A SUFFICIENT AIR GAP FOR DISHWASHER AND ICE MACHINE DRAINS. Correct By: 03-Dec-2017

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Food storage containers are not properly labeled. BULK FOOD CONTAINERS NOT LABLED WITH COMMON NAME.
Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.4(B) / Cutting surfaces.
The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. OBSERVED SEVERELY SCORED CUTTING BOARDS THAT WERE NO LONGER A SMOOTH AND EASILY CLEANABLE SURFACE.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Single-service or single-use articles, kitchenware, or tableware were not handled, displayed, or dispensed properly. OBSERVED DISHWASHER EMPLOYEE HANDLING DIRTY UTENSILS LOADING DISHWASHER AND THEN UNLOADING CLEAN UNTENSILS WITHOUT WASHING HAND BETWEEN TASKS.
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWASHING SIGNS IN EITHER MENS OR WOMENS RESTROOMS USED BY FOOD EMPLOYEES.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
