[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. OBSERVED CONDENSATION AND MOISTURE AT BASE OF REACH-IN FREEZER DOOR. REACH-IN FREEZER MAY BE IN DISREPAIR.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Utensils, thermometers, and/or pressure guages were not kept in good repair or calibration. OBSERVED THAT TEMPERATURE DISPLAY AT REACH-IN FREEZER WAS NOT ACCURATE.
(1) Utensils shall be maintained in a state of good repair or shall be discarded; (2) Food temperature measuring devices shall be calibrated as necessary to esnure their accuracy; (3) Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Observed improper draining of condensation and other nonsewage liquids. OBSERVED CONDENSATION AND MOISTURE AT THE OF DOORWAY OF REACH-IN FREEZER AND ON PANELS.
Condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law.
3717-1-05.4(N) / Covering receptacles.
Observed waste receptacles that were not covered. OBSERVED WASTE RECEPTACLE THAT WAS FULL AND NOT COVERED.
Receptacles and waste handling units for refuse, recyclables, or returnables shall be: kept covered inside the FSO or RFE if they contain food residue and are not in continuous use, and after they are filled; or with tight-fitting lids or doors if kept outside.
3717-1-05.4(R) / Removal - frequency.
Repeat Observed an accumulation of refuse, recyclables, or returnables. OBSERVED WASTE RECEPTACLE AT ENTRANCE WAY FULL AND NOT REMOVED AT PROPER FREQUENCY.
Refuse, recyclables, or returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
The floor and wall junctures are not properly coved or closed, or floor drains were not provided. OBSERVED BASE COVING IN MEN'S RESTROOM THAT LOST ADHESION.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. TO PREVENT THE ENTRY AND HARBORAGE OF INSECTS THE PIC SHALL REPAIR OR REPLACE BASE COVING.

The floor and wall junctures are not properly coved or closed, or floor drains were not provided. OBSERVED HANDSINK IN MEN'S RESTROOM THAT WAS NOT CAULED/SEALED.
In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. TO PREVENT MOLD GROWTH FROM MOISTURE BUILD-UP THE PIC MUST CAULK/SEAL HANDSINK.

Observed a build-up of dirt and debris. OBSERVED DUST BUILD-UP AT VENTALATION IN MEN AND WOMEN'S WOMEN'S RESTROOM.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
