Non-food contact surfaces of equipment are unclean. OBSERVED TRAYS USED TO WARM UP INDIVIDUAL FOOD CONTAINERS ARE SOILED AND ON THE 2 COMPARTMENT SINK. PLEASE CLEAN AND PUT AWAY TRAYS.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. OBSERVED THE 2 COMPARTMENT SINK WAS SOILED DUE TO LACK OF USE IN THE LAST FEW MONTHS. PLEASE THOROUGHLY SCRUB OUT THE SINK.

Repeat No protective shielding on lights. OBSERVED SEVERAL FLUORESCENT LIGHTS WITHOUT LIGHT SHIELDS AND THE FEW THAT DO ARE LACKING END CAPS FOR THE LIGHT SHIELDS.
