Floors, walls, and/or ceilings not smooth and easily cleanable. The ceiling in the kitchen is not a smooth and easily cleanable surface. Replace the one ceiling tile as observed to be damaged. Note: CDPH informed by staff how the one remaining ceiling tile is in the process to be completed to fully restore a smooth and easily cleanable surfacecharacteristic.

Light intensity less than fifty foot candles in required areas. Observed one non-working florescent ceiling light tube located in the food preparation area. Properly restore the light intensity to at least fifty foot candles by the replacement of the observed non-working florescent ceiling light tube which is located in the food preparation area.
