Critical PIC unable to demonstrate knowledge of foodborne disease and the significance of maintaining time/temperature controlled for safety food.

3717-1-02.4(B)(1) / PIC: Demonstration of Knowledge - No Critical Violations
Critical PIC unable to demonstrate knowledge by having no critical violations.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food not properly discarded when required. Observed various food items,stored and held in the refrigerated units beyond the date clearly marked on theitems. To prevent spoilage and food borne illness do not maintain TCS foods in the refrigerated units beyond the dates clearly marked on the food. These items were discarded at inspection to include turkey bacon, chopped ham.

Facility does not have an employee with manager certification in food protection. Ensure that the person in charge has managers certification.

3717-1-03.4(C) / Thawing - temperature and time control.
TCS foods not properly thawed. Observed food items being thawed at room temp. These items were place under cold running water.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed broken seals in various coolers, repair or replace the gaskets to maintain proper cooling efficiency.

3717-1-04.5(A)(2) / Cleanliness of food-contact surfaces of cooking equipment and pans.
Cooking equipment or pan surfaces unclean. Observed encrusted grease on cooking equipment, clean and maintain.

3717-1-05.4(Q) / Cleaning receptacles.
Waste receptacles or handling units not cleaned and maintained. Observed new plastic liners placed in soiled receptacles in the food prep area. Clean receptacles prior to placing liners in them and to minimize attracting insects and rodents.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Repair/replace broken tiles at the dishmachine.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Clean floor wall and ceiling surfaces throughout. Clean underneath and behind equipment removing observed food spillage, dirt and debris. Clean the facility at a frequency to prevent insect or vermin attraction.
