| FACILITY HAD OPENED WITHOUT A REINSPECTION FOR THEIR COMPLIANCE INSPECTION. IN ADDITION, THE FACILITY HAS RECEIVED A COMPLAINT FOR OVERFLOWING TRASH AT THE DUMPSTER AND MICE IN THE FACILITY. FACILITY HAS NO TRASH BY THEIR DUMPSTER. HOWEVER THERE IS UNUSED EQUIPMENT AND FURNITURE BY THE BUILDING WHICH THE FACILITY HAS BEEN GIVEN 2 WEEKS TO SELL, ORGANIZE, OR THROW AWAY. OBSERVED NO PEST ISSUES IN THE FACILITY AT THE TIME OF THE INSPECTION. THE FACILITY HAS REPAIRED OPENINGS ON THE EXTERIOR DOOR AND ON THE PERIMETER OF THE BUILDING. |
